Enhanced Care Management
The Enhanced Care Management (ECM) program is the newest Medi-Cal benefit. Our ECM aims to provide a whole-person approach to care addressing the clinical and social needs of complex Medi-Cal managed care patients. Arroyo Vista assigns a Lead Care Manager to each ECM enrolled patient with the overall goal to provide comprehensive care coordination and achieve better health outcomes by:
Improving care coordination
Integrating services
Facilitating community resources
Addressing social determinants of health
Improving health outcomes
Decreasing inappropriate utilization and duplication of services
