Enhanced Care Management

The Enhanced Care Management (ECM) program is the newest Medi-Cal benefit. Our ECM program aims to provide a whole-person approach to care addressing the clinical and social needs of complex Medi-Cal managed care patients. Arroyo Vista assigns a Lead Care Manager to each ECM enrolled patient with the overall goal to provide comprehensive care coordination and achieve better health outcomes by providing support with:

Medical and social service appointments
Medication Management
Transition of care after hospital discharge
Community programs and social services applications, including housing, transportation, in-home support services, and much more!
